Hospital Visitations/Prayer

We consider it a privilege to pray for and comfort family and friends who are scheduled to undergo surgery, hospitalized for an extended period, or have suffered an accident or injury. Please complete the form below and a pastor will be in contact with you.

I acknowledge that I am voluntarily submitting my personal information to Father's House Family Church.

I further acknowledge that I give permission to Father's House Family Church to maintain my personal information on their database and use my personal information for the purposes of contacting me for activities related to Father's House Family Church.
